While agentic AI might seem like a singular entity, it actually functions through five interconnected layers:
1. **Power Requirements**: Similar to all cloud computing frameworks, robust electrical infrastructure is essential to power the AI agents. 2. **Infrastructure/Hardware**: Data centers springing up globally are the repositories for the vast amounts of data that AI agents utilize.
3. **Network Layer**: It’s vital to have advanced security measures in place to protect sensitive data.
4. **Data Layer**: The intricate chain of protocols is necessary for effective digital data transmission.
5. **Application Layer**: The various forms in which users engage with AI agents can differ significantly.
Users must understand how these layers interrelate to identify and address issues effectively. Furthermore, reassessing the potential vulnerabilities at each layer is essential for responsible utilization.
Defining Responsible Use
Discussions around responsible AI often focus on the network, data, and application layers, as these are the ones users interact with most frequently. Security breaches can occur even in well-protected organizations if the data layer isn't managed correctly. For example, if an AI learns to equate a patient's pain rating of 10 with "severe pain" without proper context, it can lead to misleading documentation and consequently harm clinician trust and organizational credibility.

Effective governance in AI for healthcare requires more than just compliance with a single standard or certification.
Adopting a comprehensive strategy is vital. Industry benchmarks such as the National Institute of Standards and Technology’s (NIST) Risk Management Framework and insights from the Open Worldwide Application Security Project (OWASP) provide valuable guidance for navigating AI risk and security. Compliance with HIPAA is crucial for every healthcare AI application. Initiatives like the Coalition for Health AI, which I am part of, aim to solidify principles of agentic AI into standardized best practices across the sector.
However, having frameworks isn’t sufficient on its own. The most effective organizations are weaving these standards into their governance models, tailoring them to their specific workflows, patient demographics, and risk assessments, rather than simply fulfilling compliance requirements. The objective is to cultivate trust that elevates AI from a potential liability to a lasting asset.
Thoughtful Implementation Over Hastiness
Healthcare providers cannot ignore the importance of agentic AI, yet they must also refrain from deploying it without adequate infrastructure for responsible use. Finding the balance between overanalyzing—leading to stagnation—and underanalyzing—resulting in reckless deployment—is crucial for strategic success.
Agentic AI serves as a bridge uniting predictive, generative, and autonomous systems. Establishing this bridge sustainably requires active human involvement to set, monitor, and enforce safety parameters over time.
